The candidate shall enclose two true copies of:
- SSLC / SSC Mark Cards (Class 10)
- II Year PU Examination Certificate / Mark Card (Class 12)
- Intermediate Pre-Professional / Marks Card
- I & II Year (Diploma Pharmacy Marks Card / B-Pharma Mark Card)
The candidate must also submit:
- Conduct and Character Certificate from the last attended institution.
- Eligibility Certificate
- Income Certificate
- Originals must be produced on the date of interview.
Transfer Certificate (TC) and Migration Certificate must be submitted.
The candidate must attach:
- Indian Postal Order or Demand Draft (D.D.) for Rs. 300/- in favor of National Pharmacy Academy as registration fee for admission.
If the candidate is admitted to the said course, he/she shall also have to submit:
- Original 10+2 Marks Card, Transfer Certificate, and Migration Certificate within the specific date.
